You may request access to or obtain a copy of your records maintained by Sunrise Community Support Services LLC, subject to applicable legal requirements.

You may request amendments or corrections if you believe information in your record is incorrect or incomplete. You may also request restrictions on certain uses or disclosures of your health information. While we will consider all reasonable requests, we may not be required to agree to every requested restriction.

You may request confidential communications and ask that we contact you in a specific way or at a specific location to help protect your privacy. Reasonable requests for confidential communications will be accommodated whenever possible.

You may request an accounting of certain disclosures of your health information made by Sunrise Community Support Services LLC, as permitted by law. You may also ask questions about how your information is used, disclosed, and protected.

You may file a privacy complaint with Sunrise Community Support Services LLC if you believe your privacy rights have been violated. Filing a complaint will not result in retaliation or affect your care or services. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. How We Use Information

We may use and disclose your protected health information to provide care, coordinate services, communicate with your physicians and other authorized providers, obtain payment, and operate our agency. We may also use or disclose information when required or permitted by law, including for public health activities, health oversight, safety concerns, and certain government functions.

We are required by law to maintain the privacy of your protected health information, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect. If a breach of unsecured protected health information occurs, we will provide notification as required by applicable law.

We may share information with family members, caregivers, or personal representatives involved in your care when appropriate and permitted by law. We may also disclose information for appointment reminders, treatment alternatives, benefits-related services, and other healthcare operations.


Certain disclosures may be made without your written authorization when required by law, including reporting abuse or neglect, responding to court orders, assisting law enforcement in limited circumstances, supporting workers’ compensation matters, or helping avert a serious threat to health or safety.

We do not use or disclose your protected health information for purposes that require authorization unless you or your personal representative provide that authorization in writing.
